Please only book your appointment once you have received and completed the liver function test (it comes in the post).
You will also need:
- A Scan of any diagnosis – or proof of symptomatic problems (if possible).
- A photograph or scan of your photographic ID such as a drivers license or passport. (OR a selfie holding a household bill).
- A photograph of the completed liver test.
